This Notice of Privacy Practices applies to GYN PATH Services, Inc. (“GYN PATH,” “we,” “our,” or “us”). We are committed to protecting the privacy of your medical information and following applicable federal and state privacy laws.
This notice explains how we may use and disclose protected health information about you, how you can access this information, and what rights you have regarding your health information.
Our Responsibilities
GYN PATH is required by law to:
- Maintain the privacy and security of your protected health information
- Provide you with this notice of our legal duties and privacy practices
- Follow the terms of the notice currently in effect
- Notify you if a breach occurs that may have compromised the privacy or security of your information
We will not use or share your health information other than as described in this notice unless you give us written permission. If you give us written permission, you may revoke that permission in writing at any time, except to the extent we have already relied on it.
How We May Use and Disclose Your Health Information
For Treatment
We may use and disclose your health information to provide laboratory testing and related services. For example, we may send test results or pathology reports to the healthcare provider who ordered your test so that provider can use the information in your care.
For Payment
We may use and disclose your health information to bill and collect payment for laboratory services. For example, we may provide information to your health insurance plan, Medicare, Medicaid, or another payer to process a claim or determine benefits.
For Healthcare Operations
We may use and disclose your health information for activities necessary to operate our laboratory and provide quality services. These activities may include quality review, compliance, training, customer service, billing support, business planning, auditing, and other administrative functions.
Business Associates
We may share health information with companies or individuals who perform services on our behalf, such as billing, technology support, payment processing, legal, accounting, or administrative services. These business associates are required to protect your information and use it only as permitted by law and their agreement with GYN PATH.

We may use or disclose your health information without your written authorization in certain situations, including:

If you authorize us to use or disclose your health information, you may revoke that authorization in writing at any time. Your revocation will not affect actions we have already taken based on your authorization.
Your Rights Regarding Your Health Information
You have the following rights regarding health information we maintain about you. To exercise these rights, you may be required to submit a written request to GYN PATH.
Right to Inspect and Copy
You have the right to inspect or request a copy of certain health information we maintain about you. We may charge a reasonable fee for the cost of copying, mailing, or other supplies. In some limited circumstances, we may deny your request as permitted by law.
Right to Request an Amendment
If you believe health information we maintain about you is incorrect or incomplete, you may request that we amend the information. Your request must be made in writing and include a reason supporting the request. We may deny your request in certain circumstances, such as when the information is accurate and complete or was not created by GYN PATH.
Right to an Accounting of Disclosures
You have the right to request a list of certain disclosures we have made of your health information. This accounting does not include all disclosures, such as disclosures made for treatment, payment, healthcare operations, or those made with your authorization.
Right to Request Restrictions
You have the right to request restrictions on certain uses and disclosures of your health information. We are not required to agree to every request, except in certain circumstances required by law, such as when you pay out of pocket in full for a healthcare item or service and request that information not be shared with your health plan for payment or healthcare operations purposes.
Right to Request Confidential Communications
You have the right to request that we communicate with you in a certain way or at a certain location. For example, you may ask that we contact you by mail at a specific address. We will accommodate reasonable requests as required by law.
Right to a Paper Copy of This Notice
You have the right to receive a paper copy of this Notice of Privacy Practices at any time, even if you have agreed to receive it electronically.
Right to Be Notified of a Breach
You have the right to be notified if a breach occurs that may have compromised the privacy or security of your protected health information.
Right to File a Complaint
If you believe your privacy rights have been violated, you may file a complaint with GYN PATH or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.

GYN PATH may change the terms of this notice at any time. Any revised notice may apply to health information we already maintain as well as information we receive in the future. The current notice will be posted on our website and will include the effective date.
